This Privacy policy governs your access to and use of the website and mobile application (collectively, the “Site”) that link to or reference this Privacy Policy. This Privacy Policy governs data collection and usage on the Site, and, by accessing or using the Site, you are agreeing to this Privacy Policy and entering into a legally binding contract with CareQuorum, LLC (“CareQuorum”). CareQuorum offers the Site, including all information, tools, and services available from the Site, to you, the user, conditioned upon your acceptance of all the terms, conditions, policies, and notices stated here. Throughout this Privacy Policy, the terms “we,” “us” and “our” refer to CareQuorum.

Collection of Your Personal Information

CareQuorum may collect personally identifiable information, such as your email address, name, home or work address, and telephone number via the Site. CareQuorum also may collect anonymous demographic information, such as your ZIP code, age, gender, preferences, interests, and favorites.

CareQuorum may automatically collect information about your computer hardware and software such as: your IP address, browser type, domain names, access times and referring website addresses. If collected, this information can be used by CareQuorum for the operation of its services, to maintain quality of its service, and to provide general statistics regarding use of the Site.

CareQuorum is not responsible for the privacy statements or other content on websites outside of the Site.

CareQuorum keeps track of the pages on the Site its users visit, in order to track general Site performance, review the Site’s overall effectiveness and determine which areas of the Site are the most popular.

The transmission of information over the internet is inherently insecure and we cannot guarantee the security of data sent over the internet. CareQuorum recommends that you use complex passwords for any accounts you have with us, and, if you believe your interaction with us is no longer secure, please notify us by email at contactCQ@carequorum.com.

Children Under 18

The Site is not intended for children under 18 years of age, and CareQuorum does not knowingly collect information from children under 18. If you are under the age of 18, do not: use, or provide any information on, the Site; use any of the interactive or public comment features of the Site; or otherwise provide any information about yourself to us, including your name, address, telephone number, email address or any screen name or user name you may use. If CareQuorum learns we have collected or received personal information from a child under 18, we will delete that information. Your California Privacy Rights

CareQuorum does not currently recognize or respond to browser do-not-track signals. Instead, CareQuorum adheres to the standards set out in this Privacy Policy and offers the opt-out choices described above.

CareQuorum will occasionally update this Privacy Policy to reflect company and customer feedback. CareQuorum encourages you to periodically review this Privacy Policy to be informed of how CareQuorum is protecting your information.
